Your GATE Plan

Currently:

  • 1 in 3 of us will develop some form of dementia in our life time
  • 65% of the population will die intestate (without a will)
  • 79% of people don’t have any written plans for their end of life
  • 100% of us will die!

We spoke to lots of people about what was stopping them planning for the end of their life and, as a result, we developed the GATE Plan. This impartial planning tool is designed to help you start thinking about, talking about and recording your life and your wishes for the end of your life, sooner rather than later.

Forward planning is a healthy, positive and a responsible thing to do.

You can print either the whole Plan or just the pages you want from here.  We ask you for your postcode and age before you print because we use this information to help demonstrate use and secure further funding.  Rest assured that you cannot be identified and no other personal details are asked for, or kept.

This Plan is designed to help you and give you some ideas.  It is not about medication or medical treatment; it is about your whole life which is much bigger than that. It is your life; you decide how you want to record it, this pack is just a framework with some suggestions.  Put extra bits in, leave other bits out.  Don’t be put off by the range of topics covered.  Your GATE Plan should be a living document. Complete as much or as little as is relevant to you, it does not matter how much or how little you do.  Just one written wish, just one conversation is a positive step.
